Prepare the necessary files and documents:

  • Copy of 5th grade report card for SMP registrants.
  • Copy of 8th grade report card for SMA registrants.
  • Copy of Birth Certificate.
  • Copy of Family Card.
  • Copy of Parent’s ID Card.
  • Copy of BPJS/Other Insurance.
